 Hello! I’ve been a nurse for nearly 26 years, and for the past 16 years, I’ve been honored to care for patients as a certified Family Nurse Practitioner. I also hold certification as a Psychiatric Nurse Practitioner, with 8 years of experience supporting mental health and emotional well-being.

My background includes women’s health, urgent and acute care, emergency medicine, primary care, addiction treatment, and psychiatry. I have a special interest in chronic disease management and believe that the best care happens when we combine up-to-date medical guidelines with each person’s unique lived experience. I listen deeply, take time to understand, and work with my patients to create care plans that truly fit their lives.

Outside of work, I’ve been happily married for 22 years and am a proud mom of two daughters in college and a son in high school. I love learning new things, cooking, binge-watching TV series, laughing, and spending time with my French bulldog.

 I am Lindsay Alvarado, a board-certified Family Nurse Practitioner through the American Association of Nurse Practitioners (AANP). I obtained my nursing degree at Seward County Community College and spent many years caring for patients in labor and delivery, emergency medicine, and medical-surgical settings before continuing my education at Walden University, where I earned both my Bachelor of Science in Nursing and Master of Science in Nursing. Since 2020, I have provided comprehensive family practice care for patients of all ages—from newborns to seniors. My experience includes managing acute illnesses, performing well-child exams, supporting women’s and men's health, preventative care, and treating chronic conditions. In addition to clinic work, I have five years of experience in emergency medicine. Outside of my work, I’m a proud wife and mother who knows firsthand how much accessible, compassionate healthcare means to families. I’m grateful to be part of a close-knit community and am deeply committed to advocating for the health, education, and well-being of those around me.

 Tara Jones is a Registered Respiratory Therapist with over 24 years of healthcare experience. As the manager of Main Street Clinic, she leads a compassionate team of medical and mental health providers dedicated to making care accessible, personal, and rooted in community.

A former clinical instructor and active member of the Elkhart Chamber of Commerce and local coalitions, Tara is passionate about improving health outcomes and building strong relationships across the community. Known for her friendly nature and ability to connect with anyone, Tara brings joy, empathy, and genuine curiosity to every conversation. She feels deeply for others—sharing in their sadness, celebrating their wins, and always ready to laugh and listen.

Tara lives in Elkhart with her husband, KC, and their two children, Grady and Kamryn. Her leadership is grounded in kindness, experience, and a heartfelt commitment to the people she serves.

Hi, I'm Crystal Fleming-wife, mom of four boys, and proud lifelong resident of Elkhart.  I've been working in the medical field since 2008 and currently serve as an Advanced EMT for Morton County.  I have a heart for helping others, and I love being there when people need support the most.

Outside of work, I stay busy in a few different ways! I teach kids' classes on Wednesday nights at our church and also serve on the Board of Education for USD 218-both roles that let me give back to the community that means so much to me.

On the weekends, I enjoy relaxing and spending time with my family, especially playing cornhole in the backyard.  Faith, family, and community are what ground me-and I wouldn't have it any other way.

 I’m known for being organized, responsive, and quick to act when something is a priority for my patients—because your health matters, and so does your time.
